Re-issue of Passports in lieu of lost/ stolen /damaged passort

[Normal: 36 pages & Jumbo: 60 pages)

 

As per instructions in general guidelines, printout of duly filled online application form available at https://embassy.passportindia.gov.in/, should be submitted at VFS Centre. All particulars in the application should tally with those in the current passport. If there is a change in personal particulars, the requisite supporting document must accompany the application.

Documents required:

  1. Duly completed and printed application form (https://embassy.passportindia.gov.in)
  2. Two white background colour photographs with size 2” x 2” (51mm x 51mm) (Please refer to the photo specification mentioned in general guidelines),
  3. Self-attested copies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any
  4. Self-attested copy of UK Entry visa,
  5. Self-attested copy of Biometric Resident Card (BRP) Card or Indefinite Leave to Remain (ILR)
  6. Self-attested copies of passports, UK entry visa and Biometric Resident Card (BRP) Cards of both the parents (in case of minor applicant)
  7. Annexure D to be filled by parents [Download] (in case of minor applicant)
  8. Annexure C in case of single parent [Download]. If child’s custody order is not available, Annexure C must be notarized. In case of single parent, the application will not be accepted under ‘Tatkaal’ scheme. (in case of minor applicant)

  12. Letter addressed to Passport Officer, explaining the circumstances under which the passport was lost/ stolen/ damaged
