Reissue of passport in case of change of name

 

No Tatkaal application shall be accepted for reissue of passport in case of change of name. The change in name has first to be advertised in a daily newspaper, both in UK and in India at the area of residence.

The following documents are required to be submitted with the application after verification with the originals: -

(A)       Adult applicant:

Documents required:

  1. Duly completed and printed application form (https://embassy.passportindia.gov.in)
  2. Two white background colour photographs with size 2” x 2” (51mm x 51mm) (Please refer to the photo specification mentioned in general guidelines),
  3. Current Passport in original along with self-attested copies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any.
  4. Self-attested copy of UK Entry visa,
  5. Self-attested copy of Biometric Resident Card (BRP) Card or Indefinite Leave to Remain (ILR)
  6. At least two public/school documents in the desired name to ascertain that the applicant has actually changed his/her name (originals must be shown for verification)
  7. Clippings of two original newspapers or the Gazette notification of the concerned state
  8. Name Change Deed [Download]

(B)       Minor applicant:

Documents required:

  1. Duly completed and printed application form (https://embassy.passportindia.gov.in)
  2. Two white background colour photographs with size 2” x 2” (51mm x 51mm) (Please refer to the photo specification mentioned in general guidelines)
  3. Current Passport in original along with self-attested copies of first (personal particulars), last (address) pages and any other endorsement pages and visa page of the passport and additional booklets, if any.
  4. Self-attested copies of passports, UK entry visa and Biometric Resident Card (BRP) Cards of both the parents (Originals must be shown for verification)
  5. Annexure D to be filled by parents [Download]
  6. Annexure C in case of single parent [Download]. If child’s custody order is not available, Annexure C must be notarized. In case of single parent, the application will not be accepted under ‘Tatkaal’ scheme.
  7. Declaration for change in appearance for minor applicants [Download].
  8. At least two public/school documents in the desired name to ascertain that the applicant has actually changed his/her name (originals must be shown for verification)
  9. Clippings of two original newspapers or the Gazette notification of the concerned state

P.S.:-  Originals must be produced for verification at the time of submission of application and all photocopies must be self-attested.
